The 70% is not a technology failure number. Most pilots work. The model performs. The agent answers questions correctly. The automation runs. Then the pilot ends, the rollout begins, and usage drops. Months later, the AI feature exists but nobody uses it.

This is not a bug. It is a design failure that was entirely predictable and entirely preventable.

Why Pilots Always Work

Pilots are designed to succeed. The team is motivated and self-selected. The use case was chosen because it was tractable. The timeline is short enough to sustain enthusiasm. The AI does not need to compete with a deeply embedded workflow. It gets to demonstrate value in ideal conditions.

None of these conditions survive the rollout.

At rollout, the AI faces users who did not choose to participate. It competes against workflows built over years. It operates at a scale that exposes edge cases the pilot never encountered. The conditions that made the pilot succeed are gone, and no one designed for what comes next.

The Five Things That Kill Adoption at Scale

Gap 1
The Trust Problem
AI outputs need to be trusted before they get used. Trust is built through transparency, explainability, and calibration over time. A system that cannot explain its reasoning, surfaces errors without context, or occasionally produces confident wrong answers will be abandoned. Users do not give AI systems the benefit of the doubt. One high-profile failure in front of a customer, a manager, or a peer is enough to permanently lower adoption. This is an experience design problem, not a model accuracy problem. A model that is 95% accurate can still destroy user trust if the 5% fails in visible and consequential ways without warning.

The Workflow Integration Problem
AI that requires users to leave their existing workflow to access it will not get used. This is not a speculation. It is a consistent pattern across enterprise software adoption. People do not switch contexts to use tools that are not embedded where work actually happens. A separate dashboard someone has to remember to check, a new tab that competes with the tools already open, a standalone interface that requires a habit change before it can deliver value. None of these survive the competing demands of a full workday. The AI needs to be in the tool where work happens. That is a product design decision that must be made before the AI is built, not after adoption numbers disappoint.

The Incentive Problem
If existing performance metrics reward behaviors the AI would replace, people will not use the AI. The logic is rational, not resistant. A customer service agent measured on call volume will not use an AI that reduces the number of calls they handle. A salesperson measured on activity counts will not use an AI that reduces the number of activities needed to close deals. A manager measured on team headcount will not advocate for an AI that reduces headcount requirements. The metric needs to change before the behavior can change. Organizations that deploy AI without updating the incentive structures governing the users' work are deploying into a system that is designed to reject the technology.
Gap 4
The Skills Gap Problem
Using AI well is a skill. It is not the same as using software. Prompt construction, output verification, knowing when to override the AI and when to trust it, understanding the boundaries of what the AI can handle reliably. These require training that most rollouts do not provide. Most enterprise rollouts provide feature training. Here is what the tool does, here is how to access it, here is the interface. What they do not provide is judgment training. When should you trust the AI's output without verification? When should you override it? How do you recognize when the AI is operating outside its reliable range? Without this, users either over-trust the AI and make errors, or under-trust it and stop using it. Both outcomes kill adoption.

The Ownership Problem
When nobody owns the AI in the organization after the consultant leaves, the system degrades. Models become stale. Edge cases accumulate without feedback loops. The AI's performance drifts from its pilot baseline with no one accountable for maintaining it. Enterprise AI is not a product you install and forget. It requires ongoing ownership: someone accountable for monitoring output quality, collecting user feedback, routing edge cases back into the system, and making the judgment calls about when retraining or reconfiguration is needed. In the absence of a named owner with clear accountability, all of those things stop happening. The AI continues to exist. Its usefulness does not.

What Adoption-First AI Deployment Looks Like

The difference between a pilot that scales and one that does not is usually visible before the pilot begins. These are the questions that predict scaling.

  • Is there a named owner in the organization who is accountable for adoption metrics, not just deployment metrics?
  • Have the performance metrics that govern the users' work been updated to reflect AI-assisted workflows?
  • Is the AI embedded in the tool where work happens, or is it a separate product people need to visit?
  • Does the rollout plan include a trust-building phase, not just a training phase?
  • Is there a feedback mechanism that routes edge cases and errors back to the system?

These are not technology questions. They are organizational design questions. The technology is almost never the binding constraint at the adoption stage. The binding constraint is almost always one of the five gaps above, and each of those gaps has a design answer, not an engineering answer.

Organizations that ask these questions before the pilot ends are building toward a system that can scale. Organizations that ask them after the rollout numbers disappoint are in remediation. Remediation is expensive. Prevention is not.

The 12-Week Delivery Model

At Redesign, the 12-week AI build timeline includes adoption design as a core deliverable, not an afterthought. The last four weeks are specifically for adoption: change management planning, user onboarding design, performance metric alignment, and scaling playbooks.

The AI agent is production-ready at week eight. Weeks nine through twelve are about making the organization ready to use it.

This distinction matters. Production-ready and adoption-ready are not the same milestone. A system can be technically complete and organizationally unprepared. Most 70% failures are exactly that: technically complete, organizationally unprepared.

The four-week adoption phase covers the specific gaps that kill scaling. Who owns the system after deployment. How trust is built incrementally with users who did not participate in the pilot. What feedback loops are in place to catch performance drift. How performance metrics are updated to reward AI-assisted workflows. What the onboarding experience looks like for users encountering the system for the first time.

This is the difference between an AI that gets deployed and an AI that gets used.

The Problem Is Predictable. So Is the Solution.

The 70% failure rate is predictable. It is also preventable. The organizations that are getting value from AI are the ones that treated adoption as a design challenge before it was a deployment challenge.

They asked "how will this change how people work?" before they asked "which model should we use?" They identified the incentive misalignments before the rollout, not after. They designed the trust-building experience before the first user encountered the system.

The technology question has become much easier to answer. The models work. The infrastructure exists. The build timelines are short. The adoption question has not gotten easier. It requires the same organizational design rigor it always has, applied to a class of technology that most organizations are still learning how to deploy.

The 70% that fail are not failing at build. They are failing at the question they did not ask early enough.
